WHAT’S UP WITH CHRONIC LOW BACK PAIN? Spinal Manipulation’s Role

Chronic low back pain like that explained by the veteran in this case report is very prevalent and can be quite disabling. Spinal manipulation is recommended for the management of chronic low back in several guidelines backed by such groups as the American Pain Society and the American College of Pain Physicians. (2,3) With chronic low back pain, central sensitization – the effect of repeated nerve stimulation has on the central nervous system - is thought to contribute to chronicity of pain.  Spinal manipulation’s advantage may well come from its influence on this sensitization. (4) That is an advantage chronic back pain sufferers can tolerate.

SPINAL MANIPULATION BETTER FOR CHRONIC LOW BACK PAIN…REGARDLESS OF AGE

A systematic review and meta-analysis of randomized controlled trials (47 RCTs and 9211 participants) of spinal manipulation treatment for chronic low back pain described that spinal manipulation generated similar effects to other suggested therapies of chronic low back pain (like exercise and drug therapy), was better than non-recommended intervention for short-term improvement, and had limited, mild-to-moderate, momentary adverse effects. (5) Another such meta-analysis determined that spinal manipulation appeared to be a good alternative for pain relief and function improvement for chronic low back pain sufferers. (6) In a meta-analysis particularly focused on older (ages 65-91 y/o) chronic low back pain sufferers, the same deductions were found: spinal manipulation should be thought to be a beneficial treatment for the older patient with chronic low back pain for pain relief and function improvement. (7) Most spinal manipulation is delivered by chiropractors according to the National Center for Complementary and Integrative Health.
